Athletic Program Forms

For current student-athletes and their parents, there are required forms that must be completed to participate. Additionally, there are informative documents listed that discuss expectations and policies upheld by the athletic program of St Joseph High School.

FinalForms Participation Forms

St. Joseph High School partners with FinalForms, an online forms and data management service. FinalForms allows you to complete and sign athletic participation forms for your students.

Click on the Parent Playbook to get started in FinalForms. If you require any support during the process, scroll to the page bottom and click “Use Support”.

We are asking that ALL parents of athletes use FinalForms. Please Note: students may only try out for one sport per season.

Please register at: https://stjoseph-ct.finalforms.com/ and follow the prompts to create your account, create your students and sign your forms. Both parents and students are required to sign forms.

Any returning athletes, parents will have to review your information, make any updates as necessary. If no updates are needed, you will only have to sign each form once for the rest of the year.

Transportation Waiver Form

Athletes are expected to travel to and from games on the bus with their team. However, we do grant waivers for very special circumstances (i.e. you live closer to the away site than to St Joes).

Please sign, have your child's coach sign and then have your child bring the Transportation Waiver Form in person to Mr Butler or Ms Robertson directly if there is a specific game that your child cannot ride the bus back to school following a game. This form needs to be approved at least 24 hours before the game for which it applies:
