Athletic Program Forms

For current student-athletes and their parents, there are required forms that must be completed to participate. Additionally, there are informative documents listed that discuss expectations and policies upheld by the athletic program of St Joseph High School.

Participation Forms

The following 2 links - Athletic Participation Forms and a current physical form - must be completed and either submitted online or in person  and are required to participate in our athletic programs. The Athletic Participation Forms are required for each season that a student plays a sport:

2.) SJHS Sports Physical Evaluation Form (or Blue Health Record form is also acceptable in place of this form)

Student Athlete Handbook

St Joseph High School is a Roman Catholic educational institution whose primary function is to educate the whole student. The students’ spiritual and academic growth is our focus, but we recognize the value of a comprehensive athletic program to enhance their experience. We will support our teams and our athletes in a positive and caring way. We will follow all CIAC, FCIAC and Saint Joseph High School regulations, goals, and objectives. We encourage our coaches to stress the importance of physical fitness, conditioning, and safety in all athletic activities.

Transportation Waiver

Athletes are expected to travel to and from games on the bus with their team. However, we do grant waivers for very special circumstances (i.e you live closer to the away game site than to St Joe's). Please sign, have your child's coach sign and have your child bring the Transportation Waiver Form in person to Mr Butler or Ms Nasser directly if there is a specific game that your child cannot ride the bus back to school following a game:
